Job Summary:

The Administrative Assistant Case Management provides administrative support to the Case Management Department by assisting with discharge planning and organizing correspondence with home care agencies, rehabilitation facilities, and skilled nursing homes for evaluations of follow-up care.

Essential Duties:

  1. Assists Case Managers with discharge planning, including contacting and organizing correspondence with home agencies, rehabilitation facilities, and skilled nursing homes for evaluations and follow-up care. Documents contacts in the medical record.
  2. Responsible for sending admission notification to payer sources as directed and faxing discharge information to payer sources as directed by Case Managers.
  3. Participates in total joint class by presenting discharge information regarding home care referrals, rehabilitation placement, and outpatient services.
  4. Maintains master calendar and schedules meetings and keeps minutes for Case Management staff meetings, Utilization Review Committee, and other committees as assigned. Responsible for keeping schedules for Case Management Department activities such as the on-call schedule.
  5. Responsible for maintaining appropriate supplies for Case Management Department.
  6. Facilitates patient signing and providing second copy of Important Notice from Medicare.
  7. Maintains tracking of Quality Assurance monitors and other information and directed by the Case Management Manager.
  8. Receives and assists visitors and phone calls. Refers and transfers visitors to the appropriate individuals.

Job Requirements


  • Associate's Degree required; Bachelor's Degree preferred
  • Certified Administrative Professional certification preferred
  • 2-4 years of related experience required, 4-6 years preferred
    • OR Preferred minimum 1 year of experience as an administrative assistant in a healthcare organization
